目的: 本研究通过对比医疗机构与长护机构在护理、康复等服务项目、价格、人力成本等方面的差异,为完善长护服务价格机制提供意见与建议。方法: 整理分析38家基本医疗保险定点医疗机构、29家长期护理保险护理服务机构开展的护理项目、价格、人力成本等内容,总结对比医疗机构与长护机构在护理分级标准、服务项目分类和内容、价格及人力成本等方面的差异。结果: 在公立医院政府指导价和养老机构市场调节价的不同定价机制下,长护机构与医疗机构服务项目价格呈现较大差异。讨论与建议: 推行“支付标准–服务项目价格”二元机制,以长护险支付标准引导服务合理定价;实施弹性监管,尊重长期护理服务市场规律;深化医疗机构护理服务价格调整,构建 “医疗–长护”价格衔接机制;建立分级服务标准,强化服务连续性。

Objective: This study compares the differences of medical institutions and long-term care insurance institutions in service items, prices, and labor costs of nursing and rehabilitation, to provide suggestions and recommendations for improving the pricing mechanism of long-term care services. Methods: Nursing items, prices, and labor costs from 38 medical insurance designated institutions and 29 long-term care insurance designated institutions were analyzed to summarize differences in nursing grading standards, service item classification and content, prices, and labor costs between the two types of institutions. Results: Service prices vary significantly due to different pricing mechanisms: government - guided prices in public hospitals and market - regulated prices in elderly care institutions. Discussions and Suggestions: We should implement a dual mechanism of "payment standard-service price" and guide reasonable pricing of services based on long-term care insurance payment standards, conduct flexible supervision and respect the laws of the long-term care service market, deepen the adjustment of nursing service prices in medical institutions and establish a price linkage mechanism between medical institutions and long-term care insurance institutions, and set graded service standards and strengthen service continuity.
