目的: 本研究对典型城市流动人口常住地城乡居民医保参保门槛的相关政策进行文本梳理与分析,对比不同城市政策差异和创新举措,为参保政策的完善提供建议。方法: 在全国范围内选取89个城市,检索各城市及其所在省份发布的2025年城乡居民医保参保政策文件,采用内容分析法对政策文本进行内容提取和统计分析。结果: 大多数被调查城市已放开流动人口常住地参保的户籍限制,流动人口持居住证可参加城乡居民医保。个别大型或超大型城市对流动人口常住地参保仍存在户籍限制或其他参保门槛,且主要集中于部分随迁儿童和随迁配偶。结论: 在存在参保门槛的大型或超大型城市,建议以外地户籍的中小学生、学龄前儿童为突破口,逐步扩展到外地户籍的随迁配偶,有序推进流动人口常住地城乡居民参保政策的落地。

Objective: This study aimed to review and analyze policies regarding the enrollment thresholds for migrant populations to participate in the urban and rural residents' medical insurance in the place of usual residence in typical cities. It compared policy differences and innovative measures across cities to provide suggestions for improving enrollment policies. Methods: A total of 89 cities were selected nationwide. Relevant policy documents on the enrollment of urban and rural residents' medical insurance for 2025, released by the surveyed cities and their provinces, were retrieved from their official websites and WeChat official accounts. Content analysis was used to extract and statistically analyze the policy texts. Results: Most surveyed cities had removed household registration restrictions for migrant populations to enroll in medical insurance in their place of usual residence, allowing them to participate in urban and rural residents' medical insurance with a residence permit. However, a few large cities or megacities still maintained household registration or other restrictions for migrant populations, focusing on accompanying children and spouses. Conclusions: It was recommended to take enrolling primary and secondary school students and preschool children with non-local household registration in large cities or megacities into the local urban and rural residents' medical insurance as a breakthrough point, gradually expand to the enrollment of accompanying spouses with non-local household registration, and steadily advance the implementation of urban and rural residents' medical insurance enrollment policies for migrant populations in the place of usual residence.
