摘要

目的: 分析进入新时代以来我国医疗保障政策对家庭经济风险的保护效果。方法: 利用范畴综述法,以“家庭经济风险”“灾难性卫生支出”和“致贫性卫生支出”等为主题词,在Web of Science和中国知网上检索相关文献12754篇,通过筛选最终纳入相关研究62篇。结果: 新时代以来我国公平适度的待遇保障机制基本建立,医保支付方式改革全面推行,医疗保障与医药服务协同发展持续推进,医疗保障政策降费效果显著,城乡家庭经济风险保护水平持续提高。结论: 进入新时代以来我国医疗保障政策有效降低了家庭灾难性卫生支出和致贫性卫生支出的发生率,显著改善了居民家庭经济风险保护水平。建议：进一步提高弱势群体待遇保障水平;深化医保支付方式改革,加强医疗服务行为及效果监管评价。

Abstract

Objective: This paper aims to study the household economic risk protection effect of China's healthcare security policies since the new era. Methods: Based on the scoping review method, with "economic risk of households", "catastrophic health expenditure" and "impoverishing health expenditure" as the main topics, and Web of Science and CNKI as the data sources, 12754 relevant documents were systematically retrieved. Through screening, 62 relevant studies were eventually included. Results: Since the new era, fair and moderate benefit guarantee mechanism was basically established, the reform of medical insurance payment method was comprehensively implemented, the coordinated development of healthcare security and medical services was continuously promoted, healthcare security policies have achieved remarkable results in cost reduction, and the level of economic risk protection for urban and rural families has been improved continuously. Conclusion: China's healthcare security policies significantly reduce the incidence of household catastrophic health expenditure and impoverishing health expenditure. It is necessary to focus on improving the benefit guarantee of vulnerable groups, deepening the reform of medical insurance payment method and strengthening the supervision of medical service behaviors and effects.
