江苏省张家港市于2022年10月起开展DRG试点工作。为保障医保基金安全,张家港市医保部门探索DRG支付下基金监管新模式,建立并完善“医保智慧眼”智能监测监管平台,深入挖掘病案数据在监管医疗机构违规行为中的运用价值。本文首先分析DRG支付下县域医疗机构提供住院服务时主要违规行为的表现形式及原因,阐述“医保智慧眼”智能监测监管平台如何通过挖掘病案数据实现对医疗机构违规行为的监管模型建构、运行流程及效果评价,总结目前智能监管的不足并提出改进建议,为其他地区推进DRG支付下医保基金智能监管提供可复制推广的经验。
Abstract
Zhangjiagang City, Jiangsu Province began to carry out DRG pilot work in October 2022. In order to ensure the security of medical insurance funds, Zhangjiagang healthcare security departments explored a new model of fund supervision under DRG, established and improved the Medical Insurance Smart Eye intelligent monitoring and supervision platform, and dug into the application value of medical record data in monitoring violations of medical institutions. This paper analyzes the manifestations and causes of major violations when county medical institutions provide inpatient services under DRG payment, expounds how the Medical Insurance Smart Eye intelligent monitoring and supervision platform realizes the construction of supervision model, operation process and effect evaluation, summarizes the shortcomings of current intelligent supervision and puts forward suggestions for improvement, providing a replicable experience for other regions to promote the intelligent supervision of health insurance funds under DRG payment.
