目的: 以人工晶体集采为例,深入分析上海市某三甲医院执行高值医用耗材集中带量采购政策的效果及存在问题,对医疗服务效率变化进行评价,并据此提出相应对策。方法: 通过文献研究法、霍恩-米特模型、“投射-实施后”对比分析法、案例研究法和访谈研究法对政策实施效果进行分析探讨,总结规律方法,为高值医用耗材集中带量采购政策提供借鉴和参考。结论: 政府要持续推进集中带量采购政策,保障政策实施;医疗机构要积极配合政策的实施,加强补偿机制建设,优化医疗服务价格结构,保证临床产品多样性;医保支付方要充分发挥医保的引导作用,缓解费用支出压力。
Abstract
Objective: Through in-depth analysis of the effects and problems of the centralized procurement policy of high-value medical consumables in a first-class a hospital in Shanghai, the paper evaluates the efficiency change of medical service, and puts forward suggestions accordingly, taking the centralized procurement of intraocular lens as an example. Methods: Through the literature research, Horn-meter model, projection-after implementation comparative analysis, case study and interview, we summarize rules and methods to provide a reference for the implementation of centralized procurement policy of high-value medical consumables. Conclusion: The government should continue to promote and guarantee the implementation of centralized procurement policy. Medical institutions should actively implement the policy, strengthen the construction of compensation mechanism, optimize the price structure of medical services and ensure the diversity of clinical products. Medical insurance departments should give full play to the guiding role of medical insurance and relieve the pressure of expenditure.
