目的:了解阿尔茨海默症患者住院服务利用情况,分析患者住院服务利用及费用影响因素,为优化阿尔茨海默症住院服务利用提供科学依据。方法:数据源于全国基本医疗保险医疗服务利用调查数据库的阿尔茨海默症患者住院信息,采用统计描述方法分析患者基本信息和住院服务利用情况,用分位数回归法分析住院费用影响因素。结果:阿尔茨海默症患者年均住院次数1（1）次,次均住院天数17（32）天,住院费用16002（16602）元。住院天数在各分位数点上均影响住院费用,年龄在低分位数点上对住院费用有影响,区域分类、诊断分型、保险类型、医院级别在高分位数点上对住院费用产生影响。结论:阿尔茨海默症患者住院服务利用程度较高,早期发病患者的亲属应是AD筛查的重点人群,住院费用在保险类型和区域分布上存在较大差异。

Objective: The paper investigates the utilization of inpatient services for patients with Alzheimer's disease, analyzes the factors affecting the utilization of inpatient services and expenses, and provides scientific basis for optimizing the utilization of inpatient services for Alzheimer's disease.Methods: The Alzheimer's disease inpatients came from The National Survey Database on Use of Basic Medical Insurances and Medical Services, released by China Health Insurance Research Association(CHIRA). We classified patients' basic information and inpatient service utilization by statistical description methods, analyzed influencing factors of hospitalization expenses by quantile regression.Results: The frequency of hospitalizations was 1 (1) time, average length of hospital stay was 17 (32) days, and the hospitalization cost was 16002 (16602) yuan of Alzheimer's disease inpatients. Average length of hospital stay affects hospitalization cost at each quantile point, and the age at the low quantile point affects hospitalization cost. Regional classification, diagnostic classification, insurance type, and hospital level affect hospitalization cost at the high quantile point.Conclusion: Patients with Alzheimer's disease had a high degree of utilization of inpatient services, relatives of patients with early-onset disease should be the key population for AD screening, and there are big differences in the type of insurance and regional distribution of hospitalization expenses.
