摘要

目的: 评价精神专科药品集中带量采购的实施效果,为推进和完善精神专科药品集中采购提供参考。方法: 以某精神专科医院为例,收集其2019年12月—2022年8月国家集采中选药品的使用情况,包括执行中选品种数、使用量、销售金额、精神类中选重点品种约定任务量与次均费用、处方量、DDDs、门诊/住院病人次均药品费用、药品不良反应等指标,进行对比分析。结果: 各批集采任务量均超额完成,集采品种销售额较集采前明显提高,最高增幅达1269.71%;协议期与上一年同周期相比,同通用名非中选药品销售总金额明显下降,最大降幅为254.19%;中选品种处方数/非中选同通用名品种处方数的比值最大为30.88;门诊/住院病人次均药品费用逐渐下降;精神类中选品种次均费用较集采前明显下降,最大降幅为84.07%。结论: 精神专科药品集中带量采购减轻了精神疾病患者药费负担,提高了医保基金的使用效率。

Abstract

Objective: The paper evaluates the implementation effect of centralized procurement of psychotropic drugs to provide reference for promoting, improving centralized procurement of psychotropic drugs. Methods: The use of selected drugs of a psychiatric hospital from December2019to August2022were collected, compared, analyzed, including the number of selected products, the amount of use, the sales amount, the agreed task amount, average cost per time of selected key varieties of psychiatric category, the amount of prescriptions, DDDs, the average drug cost per outpatient/inpatient, adverse drug reactions, other indicators. Results: The tasks of each batch of centralized purchase were overfulfilled, the sales volume of centralized purchase varieties increased significantly, with the highest increase of1269.71%. Compared with the same period of the previous year, the total sales amount of non-selected drugs with the same generic name decreased significantly, with a maximum decrease of254.19%. The maximum ratio of prescription quantity of selected varieties/non-selected varieties was30.88. The average drug cost per outpatient/inpatient decreased gradually. The average drug cost of the selected varieties of psychiatric products decreased significantly compared with that before centralized purchase, with the maximum decrease of84.07%. Conclusion: The centralized purchase of psychotropic drugs reduces the drug costs for patients with mental diseases, improves the efficiency of medical insurance funds
