Objective: The paper attempts to investigate the current situation and willingness of rural residents’ participation in urban and rural residents’ basic medical insurance, analyze the influencing factors, and provide references for enhancing rural residents’ motivation to participate in the basic medical insurance. Methods: A household survey was conducted from November to December 2022 on 583 rural residents in City N of Fujian Province. Descriptive analysis, chi-square test, and logistic regression analysis were used. Results: A total of 485 valid samples were included, with a participation rate of 91.75% and willingness rate of 80.00%. Age and overall satisfaction with medical insurance had a significant positive effect on the current situation and the willingness of rural residents to participate in medical insurance (P<0.05). Besides, the overall satisfaction with medical insurance was affected by the reimbursement process, reimbursement amount, and satisfaction with medical staff (P<0.05). Conclusion: The willingness of rural residents to participate in medical insurance is relatively low, which can be improved by adjusting the personal contributions, promoting universal insurance coverage, optimizing the reimbursement process, and improving outpatient reimbursement limit. Primary medical institutions should strengthen personnel training to improve the quality of medical services.
