Objective: This study intends to analyze the impact of DRG payment on the cost of inpatients in Kaifeng City, Henan Province, and further verify the reduction of DRG payment on inpatients' cost in economically underdeveloped areas. Methods: A total of 208347 inpatient medical insurance patients in a tertiary hospital in Kaifeng City was selected. The impacts of DRG payment on various inpatient expenses and length of stay were analyzed by generalized linear regression, and the differences of DRG payment implementation effect among patients with different gender, age, medical insurance type and whether to perform surgery were explored by stratified analysis. Results: After the implementation of DRG payment, patients' hospitalization expenses, out-of-pocket expenses, and length of stay decreased significantly by 13.3%, 21.2%, and 14.8% (P<0.05). Stratified analysis showed that the effect of DRG payment on hospitalization cost reduction was reflected in different types of patients. Besides, the effects of DRG payment on the reduction of various hospitalization expenses of male, greater than or equal to 40 years old and surgical patients were significantly higher than those of female, less than or equal to 40 years old and non-surgical patients (all P interaction <0.05). Conclusion: The effect of DRG payment reform in Kaifeng City is remarkable. Medical institutions need to actively adjust the management mode, and the medical insurance department needs to continue to optimize the DRG implementation scheme to jointly promote the further promotion of DRG payment method.
