医保影像云是由国家医保局主导建设的云端医疗影像管理系统,旨在实现“患者可阅、同行可查、医保可核”的业务目标,破解群众跨院就医重复检查难题。本文基于系统论视角,从系统架构、运行机理与优化机制三个维度,对医保影像云业务体系的建设逻辑进行分析。研究将业务体系解构为数据采集、质控、缓存与调阅四类核心业务,阐释其“采集—质控—缓存—调阅”全链路闭环的运行逻辑;揭示该体系以“1+32”层级化架构为组织骨架、以“索引集中、数据分布”为技术底座、以医保影像云索引为流通枢纽、以“5元服务费”为运作支点、以多元化结算机制为运营保障、以数据驱动的医保治理为价值引擎的协同架构。研究发现,该体系在运行中面临数据质量治理困境、医疗机构激励不足、隐私保护与数据利用的平衡难题、运营可持续性的结构性风险四重挑战,并据此提出全链条数据质量治理、激励相容的参与机制、隐私保护法制框架、可持续运营生态与技术演进路径四个维度的优化路径,以期为医保影像云高质量发展提供理论支撑与决策参考。

The Medical Insurance Image Cloud (MIIC) is a cloud-based medical imaging management system led by the National Healthcare Security Administration, aiming to achieve the business objectives of "accessible to patients, retrievable by healthcare providers, and verifiable by medical insurance" and address the persistent problem of repeated imaging examinations across healthcare institutions. Drawing on systems theory, this paper analyzes the construction logic of the MIIC business system along three dimensions: system architecture, operation mechanisms, and optimization mechanisms. The study deconstructs the business system into four core business domains—data collection, quality control, caching, and retrieval—and elucidates the operational logic of the full-pipeline closed loop of "collection-quality control-caching-retrieval". It further reveals a synergistic architecture comprising the "1+32" hierarchical structure as the organizational backbone, "centralized indexing with distributed data" as the technical foundation, the MIIC index as the data circulation hub, the "5-yuan service fee" as the operational fulcrum, diversified settlement mechanisms as the operational safeguard, and data-driven medical insurance governance as the value engine. The study identifies four structural challenges confronting the system: dilemmas in data quality governance, inadequate incentives for healthcare institutions, the trade-off between privacy protection and data utilization, and structural risks to operational sustainability. Accordingly, it proposes optimization pathways across four dimensions—full-pipeline data quality governance, incentive-compatible participation mechanisms, a legal framework for privacy protection, and sustainable operational ecosystems with technology evolution paths—so as to provide theoretical support and policy references for the high-quality development of the MIIC.
